Thank you for participating in the Auxiliary Personnel Department Annual Member Survey. Please remember that this Survey is completely anonymous. We do not ask your name, EmpID or track your IP address.

Please feel free to be completely honest and constructive.

This time the Survey is broken into two sections, Demographics and Member Values.

This first section is Demographics, it has 10 questions. Please answer all questions. When you hit submit, you will be taken to the second section - Member Values. This section has 11 questions.
